Session I - Red Flags of a Failing Project

Most projects watch the dashboard, not the road. Leaders rely on familiar metrics and feel reassured by “green” indicators, even as meaningful warning signs develop outside the reporting frame. Dashboards capture activity, not vulnerability, and they often filter out the signals leaders most need to notice early.


This session helps leaders, assurance professionals, and oversight roles recognize the subtle patterns that indicate a project or initiative may be drifting off course long before issues appear in formal reports. Drawing on real-world examples, Matthew explains why critical conditions rarely surface in dashboards or slide decks, how routine reporting creates blind spots, and what leaders can do to surface early signals before problems escalate.

Matthew Oleniuk is a former Chief Audit Executive, senior executive, and PMP who provided oversight of large, high-stakes government projects – environments where optimistic reporting, governance complexity, and political pressure routinely obscure emerging problems. His work focused on identifying early warning signs that rarely appear in dashboards, reports, or formal assurance processes.

Today, Matthew works with executives, assurance professionals, and project leaders to strengthen how risk insight informs real decisions. His sessions challenge false confidence, expose the limits of standard reporting, and help leaders intervene earlier – before delivery issues escalate into public, costly failures.
